POLICIES/PROCEDURES
POLICY ON PRINTING OF REGISTRATION FORM (STUDENTS COPY)

  • DUPLICATE COPY FOR ANY LAWFUL REQUIREMENTS:
    Student must present his/her original Registration Form (Students Copy) and receipt of the payment for the re-printing of the Registration Form (Students Copy)
  • RE-PRINTING DUE TO PROVEN NEGLIGENCE/LOSS:
    Student must provide a notarized affidavit of loss signed by the Discipline Officer and a receipt of the payment for the replacement of the Registration Form (Students Copy).
    A copy of the affidavit of loss will be turned-over to the MIS Department for documentary purposes
  • RE-PRINTING DUE TO INCORRECT INFORMATION DURING ENROLLMENT:
    Student must endorse the 4 copies (College)/ 3 copies (High School) of Registration Form to the MIS Department and a new copy will be provided to the student.
  • RE-PRINTING DUE TO INCORRECT INFORMATION AFTER ENROLLMENT:
    Student must endorse his/her original Registration Form (Students Copy). A new copy will then be provided to the student.
